Individual Student Health Care Plan.


 

INDIVIDUAL STUDENT HEALTH CARE PLANS

Students with diabetes, seizure disorders, severe asthma, heart disease, severe allergies, or other chronic health conditions requiring monitoring who are enrolled in the Santa Rita Union School District must have a current Individual Health Care Plan signed off by the child’s physician prior to beginning initial school attendance and subsequent attendance each year.

Designated school staff members who oversee the student will receive annual training administered by a registered nurse regarding how to appropriately handle the student’s specific day-to-day health care needs and any emergency associated with their health condition that may arise.

Coordination of these services will be provided by the Director of Student Services.  Please call 443-7200 x 210 immediately upon enrollment to initiate the Individual Student Health Care Plan process and the staff training support so your child may start school as soon as possible after enrollment.
